New PatientsPlease fill out your personal and medical information first then electronically sign the three forms at the bottom:
1) Medical Disclosure Form 2) Medicare Private Contract (states Dr. Bjork won’t bill Medicare) 3) Consent to Treat THEN PLEASE SUBMIT THE ENTIRE PACKET. Press the "SUBMIT" button on the bottom right of the screen. You will then see a message "Once you've submitted paperwork, you can't change it. Do you wish to submit". After submitting the paperwork, you'll see a message confirming you've submitted the paperwork. When you close that window, you will be directed to a website to schedule your appointment. If you don't see this message, please double-check everything then press the "submit" button again. |
